The PREFER-DC dental only plan is for Students. Rates monthly are Single @ $5.95, Couple @ $9.95, and Family @ $12.95. All memberships include a one time, non-refundable processing fee of $20. *Dental Program savings are based on the 2001 National Dental Advisory Fee 80th percentile fee schedule. This is a discount program, not an insurance plan. The CAREINGTON charges are reduced fee for services performed by a participating general dentist. Any procedure not listed on the Schedule of Services is available on a fee for service basis at a 20% discount from the participating provider’s fee schedule. Consult with your provider prior to beginning any treatment. Some services, at the discretion of the general dentist, may need to be referred to a specialist. Any treatment provided by a participating specialist will be charged at a 20% reduction of the specialist’s fees for that particular case. Member is responsible for discounted fee at time of service. The program and its administrators have no liability for providing or guaranteeing service and have no liability for the quality of service rendered. Fees do not include lab costs which are the member’s responsibility. Actual rates and savings may vary based on where the customer lives.
