THE NEW SCHOOL

STUDENT ACCIDENT & SICKNESS INSURANCE PROGRAM WAIVER FORM
2009-2010 ACADEMIC YEAR

SPRING WAIVER

Welcome to the New School Student Health Insurance Online Waiver System. All degree, diploma, online only, visiting, mobility (study abroad), Lang and Parsons, graduate certificate program, graduate and undergraduate degree program non-matriculating students are automatically charged the Student Health Services Fee and the Student Health Insurance Fee. Depending on course load and status, you may be eligible to decline these student services by submitting an Online Waiver Form by the Spring waiver deadline date, February 22, 2010.

Students at the Milano branch campuses and Parsons Decorative Arts program in Washington, DC, are ineligible for the Student Health Insurance coverage.

If you need assistance while navigating this system, please contact University Health Plans at 1-800-437-6448.

Please take a few minutes to review your current policy and make sure you are adequately covered. Many HMOs provide limited coverage for students traveling away from their service area, and most employer plans will not cover dependents beyond a certain age (for instance, turning 19 or 21). If you are not adequately insured while you are at school, you will not be allowed to waive. If you are not sure, please contact your current carrier.

Staying healthy is an important part of your academic success. The New School Plan 1 - Basic Accident Plan (included with the Student Health Services Fee), and Plan 2 - Accident and Sickness Plan, have been developed especially for New School students. The Plans provide coverage for illnesses and injuries that occur on and off campus, and include special cost-saving features to keep the coverage as affordable as possible. The New School is pleased to offer these Plans.

Plan highlights include:

  • Access to over 500,000 providers in the Aetna Network
  • National prescription drug card program
  • $500,000 aggregate maximum per condition
  • Comprehensive worldwide coverage, 24 hours a day
  • Unlimited Medical Evacuation and Repatriation
  • $10,000 Accidental Death & Dismemberment

THE WAIVER DEADLINE IS FEBRUARY 22, 2010.
