Q: What is SID?
A: This is your Student Identification Number. It must be one uppercase letter followed by 8 digits.
Q: In Section A, there are two columns. Where do I put the effective date of coverage?
A: In most instances, the date will be pre-populated. If it is not, enter the effective date under the “Effective Date/Date of Event” column.
Q: I did not enroll at the beginning of the year because I was covered under another policy. However, that policy has ended. What date should I use?
A: The date of the qualifying event (i.e. the date you involuntarily lost other coverage) will be the effective date. Please include the Certificate of Creditable Coverage with the application.
Q: When can I add my dependents?
A: You can add your dependents only when there is a qualifying event. This includes date of marriage, date of birth (for newborns), or date when other coverage is involuntarily lost.
Q: Is there a deadline for enrolling my dependents for a qualifying event?
A: Yes, the deadline is 31 days from the date when other coverage is involuntarily lost.
Q: Will the cost of enrolling my dependents be pro-rated?
A: Yes, the premium will be pro-rated. Please contact University Health Plans at 800-437-6448 for a quote.
Q: Can I pay the premium for my dependents on a quarterly basis?
A: Yes, only if you are enrolling your dependent for annual coverage. Please refer to the online dependent payment policy. Please contact University Health Plans if you have additional questions.
Q: It gives me the option to “remove” coverage. Can I cancel my coverage anytime I want?
A: No. Coverage can only be cancelled under certain circumstance, including entry into the armed forces. See the plan brochure for complete details on when the coverage can be cancelled. Only use this field when it is applicable.
Q: Why do you need my Race/Ethnicity?
A: The state of New Jersey requires that this question be included on the enrollment form. However, it is optional, and the information is not needed for insurance purposes by either University Health Plans or Aetna Student Health.
Q: If I am enrolling for only myself as joint-program or optional student for annual coverage, what is my enrollment deadline date?
A: The deadline to enroll myself voluntarily is August 15, 2009.
Q: Is there a deadline to enroll for new incoming voluntary students during various enrollment terms?
A: Yes, below are the deadlines.
Fall Term Enrollment: August 15, 2009
Winter Term Enrollment: January 15, 2010
Spring Term Enrollment: March 15, 2010
Summer Term Enrollment: June 15, 2010
Q: Can I enroll myself for coverage in the middle of the year?
A: Yes, only if you are experiencing a qualifying event and enroll within 31 days from loss of other coverage.
Q: Where do I send my application and payment?
A: Please send the signed application and payment directly to University Health Plans at One Batterymarch Park, Quincy, MA 02169. Please remember to include the Certificate of Coverage letter from your other insurance carrier if you are enrolling your dependents with a qualifying event.
