CONSOLIDATED HEALTH PLANS DISCOUNT CARD

The Consolidated Health Plans Discount Card is not an insurance plan but a discount program that provides savings on services that may not be covered by the insurance plan, and offers an array of wellness and discount services. Program participants will receive a discount card in addition to the medical insurance card and can access the list of participating providers at the links provided below.

Express Scripts Prescription Discount
- Discounts range up to 35% off brand names
- CHP does not exclude any prescription type. All prescribed medicines are eligible for a discount.

Express Scripts Vision Program
As a member of the Express Scripts Vision program you can take advantage of savings on eyewear, eye exams and other services at locations nationwide.
Students can receive eye exams and choose from a wide selection of frames and lenses or contact lenses at a discount once per year per Davis Vision network provider. Here’s a sample of the savings students can enjoy:
- 15% discount on eye examinations
- Up to 40% savings on frames
- Up to 30% savings on lenses
- 10-20% discount on contact lenses
- 20% discount on non-prescription sunglasses
