DEPENDENT COVERAGE
2009-2010 Policy Year

All special students carrying nine (9) or more credits hours and degree seeking students, spouses and all their dependent children, age 18 and under, are eligible for this coverage. In order for Your dependents to be eligible for this coverage, You must be: covered under the policy; elect such coverage at the time of initial enrollment; and make the proper premium payments.

To enroll Your eligible dependents you must submit the on-line enrollment form and pay the premium by credit card.
