The Consolidated Health Plans Discount Card is not an insurance plan but a discount program that provides savings on services that may not be covered by the insurance plan, and offers an array of wellness and discount services. Program participants will receive a discount card in addition to the medical insurance card and can access the list of participating providers at the links provided below.

Express Scripts Prescription Discount
- Discounts range up to 35% off brand names
- CHP does not exclude any prescription type. All prescribed medicines are eligible for a discount.

Express Scripts Vision Program
As a member of the Express Scripts Vision program you can take advantage of major discounts on a variety of professional eye-care services and products at thousands of locations nationwide. With the Express Scripts Vision program you are eligible for immediate savings on eye-care products:
- Up to 50 percent on frames
- Up to 45 percent on bifocals
- Up to 20 percent on non-disposable contact lenses
In addition, many independent doctors of optometry and ophthalmology have agreed to discount eye exams:
- $5 of normal spectacle exam fees
- $10 off normal contact lens exam fees
The Express Scripts Vision program is administered and serviced by Cole Manages Vision and is honored at thousands of locations nationwide. You'll find providers conveniently located in Sears, JC Penney, participating Pearle Vision Centers and many other stores.
