Welcome to the UMASS-Worcester Student Accident & Sickness Insurance Program.
By January 31, 2010, your insurance company will issue a 1099-HC form for the 2009 tax year to qualifying subscribers residing in MA who were enrolled in the school health insurance plan during the 2009 calendar year. Members who have questions may call Member Service at the toll-free number on the front of their member ID card.
New Students enrolling for the UMASS-Worcester Student Accident and Sickness
insurance plan will receive an insurance card. Returning Students re-enrolling in the
school's health insurance plan will continue to use their insurance cards from last year.
