Dear Student,
The information that is accessible on this webpage describes the insurance benefits offered through Nationwide Life Insurance Company. Two insurance options are provided and it is each student’s responsibility to understand the policy benefits, exclusions, limitations and claim submission procedures. A complete description of benefits by clicking “Brochure” on the left side of this page. STUDENT ACCIDENT AND SICKNESS PLAN AND ENROLLMENT INFORMATION
- Brochure: We encourage you to read the complete description of benefits available to you through both the Student Health Services and the Nationwide Life Insurance Company Accident and Sickness Plan available on this website.
- Enrollment Information:
International Students: The Student Accident and Sickness Plan is mandatory for University of Delaware International Students here on J-1 or F-1 visas. International students will automatically be charged for the Blue Plan. If desired, an international student can follow the “Enrollment Instructions” below to upgrade to the Blue & Gold Plan and/or to add coverage for a spouse and/or child(ren.) International students who have equivalent insurance with claims processing in the United States may waive the insurance at the Office for International Students and Scholars at 413 Academy Street before September 12, 2010.
Domestic/United States Students: While enrolled at the University of Delaware, all matriculated full-time and part-time undergraduate and graduate U.S. students (and their dependents) are eligible to purchase this insurance program on a voluntary basis. Please refer to the “Enrollment Instructions” below. U.S. students are not automatically enrolled nor are they required to enroll in this insurance program.

Paying with Credit Card Online: We encourage you to take advantage of our On-Line Enrollment Management Service which allows domestic students to enroll and pay online. International students may also use the online forms to upgrade to the Blue & Gold Plan and/or add dependent(s) to their plans. To reach the form, click on the blue box to the left that says “ENROLL”. Select the option to pay by credit card. Complete all required fields and submit form. If required information is missing, your form will not be accepted. You will receive a confirmation number if your form was successfully submitted. If the form was not successfully accepted, double check that all required information was included and resubmit. If you continue to have trouble, contact University Health Plans at info@univhealthplans.com or 800-437-6448.
Paying with Check or Money Order by Mail: If you would like to pay by check or money order, you can access a printable form by clicking on the blue box to the left that says “ENROLL”. Select the link for the paper forms. Print, complete and mail the appropriate form to the address on the second page with payment for the total amount due.
DENTAL
- The University of Delaware is pleased to offer students and their dependents a dental insurance plan (Delta Dental PPO) effective September 1, 2010. You may enroll in this plan on a VOLUNTARY basis to all students; it is not required insurance. The on-line enrollment form, plan benefit highlights, and a list of network dentists can be found by going back to our homepage (www.universityhealthplans.com), clicking on “University of Delaware” and selecting the Voluntary Delta PPO Dental link.
