Stevens Institute of Technology
2010-2011 Graduate Student Health Insurance Plan
|
Stevens has selected University Health Plans (UHP) to provide a student health insurance plan effective from August 18, 2010 to August 18, 2011. This will be our 7th year with UHP as their performance in terms of value and customer service justified renewing with them. The 2010-2011 academic year insurance premium is $881. The plan has been improved by increasing the maximum from $50,000 to $75,000. Plan benefits are described in the Schedule of Benefits (SOB) at http://www.universityhealthplans.com/brochures/brochure.cgi?school_id=136.
In addition to the standard plan, there is the opportunity to purchase, on a voluntary basis, a catastrophic health insurance policy which begins coverage where the standard plan ends. This memo addresses state and federal mandates, the on-line procedure for waiving coverage, a Plan brochure, premiums, the Voluntary Catastrophic Policy, and optional Dental Insurance.

New Jersey law mandates that all full-time students have health insurance. Students are automatically enrolled in the insurance plan and a premium for coverage is added to their tuition bill unless proof of comparable coverage is furnished. Students who have comparable insurance coverage can waive the student plan on-line at www.universityhealthplans.com and selecting Stevens Institute of Technology.
The deadline to submit a waiver is September 23, 2010 (February 9, 2011 for Spring semester new admits). If you choose to waive the coverage, we recommend doing so as soon as possible. Waivers must be submitted online. No paper forms will be accepted. Students who waive the plan online will be able to print out a confirmation of their waiver request. If full-time students do not submit a waiver by the deadline, they will be automatically enrolled in the plan.
A brochure describing the Plan in detail, including a Schedule of Benefits, can be found at www.universityhealthplans.com and selecting Stevens Institute of Technology. For most graduate students, including those with F1 visas, the annual premium is $881. Coverage is also available for spouses and children of graduate students and for J-Visa Holders/Exchange Visitors and their dependents. Those rates and enrollment information for dependents can be found on the UHP website.
While graduate students with internationally-based health plans can decline this plan, we recommend that you accept the coverage. Purchasing the student insurance program assures access to local care and eliminates potential problems with plans based outside the US.
Students with no other health insurance coverage should consider purchasing the "Voluntary Catastrophic Policy." This policy, at a cost of $375, expands coverage from a maximum of $75,000 to an aggregate of $275,000 and is purchased directly from UHP. The policy description and enrollment information is available on the UHP website.
In addition to the health insurance program, Stevens is pleased to offer our students and their dependents a Dental Insurance Plan (DeltaCare). You may enroll in this plan on a voluntary basis. The on-line enrollment form, plan benefit highlights, and a list of network dentists can be found at the Stevens link on the UHP website.
Should you have any questions about the on-line waiver process, benefits, and voluntary plans please contact University Health Plans at 1-800-437-6448.
