STUDENT ACCIDENT & SICKNESS INSURANCE PROGRAM
2010-2011 Academic Year

Dear Parent or Guardian:

All students at Worcester Academy are required to be covered under a health insurance plan for the entire academic year. A student will be enrolled in the plan offered through Worcester Academy unless proof of comparable coverage is provided. Parents of all out-of-state students should confirm with their individual health plans that services will be covered if needed when a student is in school out of state. Any uncovered medical expenses are the responsibility of the parent(s).

The Academy offers a Student Health Plan that will provide worldwide protection to help cover the costs of treatment for an injury or sickness. The cost of the plan is $998 for the one-year period August 1, 2010 to August 1, 2011. Please refer to the Insurance Brochure for an outline of the plan. DENTAL

Worcester Academy is pleased to offer our students a Delta Dental insurance plan (DeltaCare) effective September 1, 2010. You may enroll in this plan on a VOLUNTARY basis, it is not required insurance. The on-line enrollment form, plan benefit highlights, and a list of network dentists can be found by linking to www.universityhealthplans.com and selecting Worcester Academy and then Dental.

VISION

Worcester Academy is also pleased to offer our students a VSP Vision Care Plan effective September 1, 2010. You may enroll in this plan on a VOLUNTARY basis, it is not required insurance.
