The enrollment deadline has been extended to 2/15/2010. The coverage period will be 3/1/2010 to 8/31/2010. The premium is not prorated.

Dear Massachusetts Students,

University Health Plans is pleased to offer students attending Massachusetts colleges and universities and their dependents a dental insurance plan (Delta Care) effective January 1, 2010. You may enroll in this plan on a VOLUNTARY basis; it is not required insurance.

The cost for the dental insurance for the period of January 1, 2010 to August 31, 2010 is:

Students and dependents who wish to enroll MUST enroll and pay the full amount by credit card on-line by midnight on December 15, 2009 to be eligible for the January 1, 2010 start date.

As part of the enrollment process, you (and any dependents you choose to enroll) must select a DELTA CARE participating dentist. Your enrollment form will not be accepted without this election. Please refer to the participating provider list on the website for a complete, up to date, list of participating providers.
