Student Accident & Sickness Insurance Program
2010-2011 Academic Year

Dear Students:

UMASS-Worcester is committed to promoting good health and meeting the health care needs of our students. A health insurance plan is critical in providing peace of mind, knowing that students can receive the services they need in the event of a sickness or injury.

Massachusetts Law mandates that all full-time and three-quarter time students carry adequate medical insurance to help cover the extra expenses of medical treatment. The UMASS-Worcester Student Accident and Sickness Insurance Plan provides coverage to students for a 12 month period, September 1, 2010 to August 31, 2011. The start date may be earlier for the first year students. We urge you to consider enrolling in this plan because it includes a local and national network of Preferred Providers and is designed to be an affordable option.

ALL UMASS-WORCESTER STUDENTS ARE REQUIRED TO EITHER ENROLL OR WAIVE THE STUDENT HEALTH INSURANCE PLAN. FAILURE TO WAIVE OR ENROLL WILL RESULT IN YOUR BEING RESPONSIBLE FOR THE INSURANCE PREMIUM.

All Students are required to complete the
OnLine Enrollment/Waiver Form

The ONLINE ENROLLMENT/WAIVER is the only
accepted process for making your insurance selection.
