VSP Vision Care

2009-2010 Academic Year

The enrollment deadline for VSP Vision Care has been extended until 2/15/2010. Any individual who enrolls from 1/16/2009-2/15/2010 will have an effective date of 3/1/2010. This will be last extended enrollment period. Premium will not be pro-rated.

Dear College/University Students,

University Health Plans is pleased to offer students attending colleges and universities and their dependents a vision insurance plan, effective January 1, 2010. You may enroll in this plan on a VOLUNTARY basis; it is not required insurance.

The cost for the vision insurance for the period of January 1, 2010 to August 31, 2010 is: Single:      $115.00 Family:      $325.00 Students and dependents who wish to enroll MUST enroll and pay the full amount by credit card on-line by December 15, 2009 to be eligible for the January 1, 2010 start date.

Once you are enrolled, no ID cards or claim forms are needed when you see a VSP Network Doctor. Tell the doctor you are a VSP member when making the appointment. If you need to find a VSP doctor, go to vsp.com or call 800-877-7195.
