VSP Vision Care

2010-2011 Academic Year

The enrollment deadline for the VSP Vision Care Plan has been extended until 9/15/2010. Any student who enrolls from 8/16/10 to 9/15/10 will have an effective date of October 1, 2010. The premium is not pro-rated.

Dear College/University Students,

University Health Plans is pleased to offer students attending colleges and universities and their dependents a vision insurance plan, effective September 1, 2010. You may enroll in this plan on a VOLUNTARY basis; it is not required insurance.

The cost for the vision insurance for the period of September 1, 2010 to August 31, 2011 is: Single:      $125.00 Family:      $353.00 Students and dependents who wish to enroll MUST enroll and pay the full amount by credit card on-line by August 15, 2010 to be eligible for the September 1, 2010 start date.

Once you are enrolled, no ID cards or claim forms are needed when you see a VSP Network Doctor. Tell the doctor you are a VSP member when making the appointment. If you need to find a VSP doctor, go to vsp.com or call 800-877-7195.
