STUDENT HEALTH INSURANCE PLAN
2014-2015 Academic Year
Brandeis University is pleased to announce that for the 2014-2015 Academic Year we have teamed up with Blue Cross Blue Shield of Massachusetts (BCBS) and University Health Plans (UHP) to provide health insurance coverage for our students. The insurance coverage is through BCBS with UHP providing administrative support for the waiver and enrollment forms, and customer service for general insurance questions.
Undergraduate and Graduate Students will be offered the same BCBS plan design, but will continue to have different premiums due to plan utilization. The Graduate Health Insurance Plan will remain the same as the 2013-2014 plan with a higher office visit copay of $20 (previously $15). The Undergraduate Health Insurance Plan will be comparable to the 2013-2014 plan design, but will be insured by BCBS (previously United/Harvard Pilgrim).
The Brandeis Student Health Insurance Plan includes a strong nationwide Preferred Provider Organization (PPO) network of providers. Highlights of the Student Health Insurance Plan include no deductible, $20 office visit copay, $50 ER copay, no copay for labs and x-rays, and 100% coverage for preventative care and hospitalizations.
Student Health Insurance Policy Period and Premiums for NEW Spring Students:

All students enrolled in 9 or more credits are required by Massachusetts State law to enroll in the Student Health Insurance or be covered by a health insurance plan with comparable coverage. To ensure compliance with Massachusetts law, students who are registered for 9 or more credits are automatically billed for the Student Health Plan, unless a waiver form is completed before the deadline date of January 5, 2015.
All international students at Brandeis are automatically enrolled in the Student Health Insurance and do not need to take any action. Please contact University Health Plans at (800) 437-6448 if you are covered by a US health insurance plan that meets healthcare reform standards and is comparable to the Student Health Insurance Plan. Please note that that Brandeis Student Health Insurance Plan meets J1/J2 visa requirements.
All domestic students enrolled in 9 or more credits must annually waive or enroll in the Student Health Insurance.
Deadline for NEW spring students to complete the waiver or enrollment form is January 5, 2015

IMPORTANT INFORMATION FOR STUDENTS TAKING A LEAVE OF ABSENCE
A. Taking a leave of absence within 30 days after the start of classes:
You are not eligible for the Brandeis Student Health Insurance Plan. Should you use your insurance prior to going on leave, you will be responsible for the full amount charged for care as your insurance will be retroactively terminated. For students starting in the fall the plan will be termed back to the effective date of the plan, which is 8/15/14. For students starting the spring the plan will be termed back to the effective date of the spring plan, which is 1/1/15.
B. Taking a leave of absence after 30 days after the start of classes:
You are eligible to remain enrolled in the Brandeis Student Health Insurance Plan, if you are enrolled in the plan at the time you take a leave of absence.
