The deadline to waive, enroll as voluntary students or dependents for the spring term one is January 15, 2010.

This letter contains important information about student health insurance for new students and for continuing students – both those who are already enrolled in the University’s Student Health Insurance plan, underwritten by Aetna, as well as those who waived participation in the University’s Student Health Insurance Plan last year.
Under University policy, all full-time UMDNJ students as well as those part-time UMDNJ students who participate in clinical experience as part of their educational programs are required to be covered by health and accident insurance. The University, in conjunction with Aetna Life Insurance Company, offers a plan which fulfills the UMDNJ insurance requirements.
This Plan offers two levels of benefits: an “in-network (preferred)” benefit and an “out-of-network (non-preferred)” benefit. Aetna has one of the largest networks of physicians, hospitals and other healthcare providers in the United States.
To find a provider, log on to www.aetnastudenthealth.com then click on “Find Your School” enter 812807 as your Policy Number. You can use DocFind® to find out whether a specific provider belongs to Aetna’s network or to find preferred providers practicing in your area.
The cost of this Plan for each student for the 2009/2010 Academic Year is $2,065. Coverage is effective August 1, 2009 through July 31, 2010. The Plan reimburses eligible non-hospital expenses at 90% for in-network (preferred care) providers and 60% for out-of-network (non-preferred care) providers. Hospital charges are reimbursed at 100% for in-network (preferred care) providers and at 70% for out-of-network (non-preferred care) providers. There is a $75 per policy-year outpatient deductible and a $500 per admission inpatient deductible. After your deductible(s) and coinsurance responsibilities reach an annual out-of-pocket maximum of $1,500 for in-network (preferred care) services ($10,000 for out-of-network (non-preferred care), then all eligible expenses will be reimbursed at 100% for the remainder of the policy year. Prescription drug co-pays and deductibles do not apply towards meeting the annual plan deductible or annual out-of-pocket maximum.
The Student Health Insurance Plan also offers a Travel Assistance provision, which includes medical evacuation and repatriation benefits and travel assistance services. These benefits are underwritten through Virginia Surety Company and through On-Call International. Such insurance is mandatory under the University policy on Students Participating in Educational Activities outside UMDNJ or its Affiliates. ID cards will be issued as soon as possible. If you need medical attention before the ID card is received, benefits will be payable according to the Policy. You do not need an ID card to be eligible to receive benefits. Prior to receiving your ID card present the provider’s office with Aetna Student Health’s Customer Service number and claims address. Once you have received your ID card, present it to the provider to facilitate prompt payment of your claims. To print a temporary ID card, enroll in Aetna Navigator your secure member only website. Once enrolled you can view your eligibility, claims, print a temporary ID card, request a new ID card, look up provider and much more. If you are a new student......
- If you are a new full-time student or a new part-time student who participates in clinical experience and pays tuition directly to UMDNJ, you will be automatically enrolled in and billed for the Student Health Insurance Plan on your UMDNJ tuition bill. You do not need to complete an Enrollment Form. However, if you wish to enroll eligible dependents, you must complete a Dependent Enrollment Form. This Form will be available at your School or you can download a Form at www.universityhealthplans.com. The completed Form and premium payment must be sent to our service provider, University Health Plans, One Batterymarch Park, Quincy, MA 02169. The deadline for enrolling dependents is August 15, 2009.
- If you are a new student who is required to be covered by health and accident insurance, but pay tuition to one of UMDNJ's partner institutions that co-sponsors a program with UMDNJ, you must complete an Enrollment Form. In addition, eligible students who are not required to be covered by health and accident insurance but wish to enroll in the Student Health Insurance Plan on a voluntary basis may do so by completing the Enrollment Form. This Form will be available at your School or you can download a Form at www.universityhealthplans.com. The completed Form and premium payment must be sent to University Health Plans, One Batterymarch Park, Quincy, MA 02169. The deadline for enrolling is August 15, 2009.
- If you are a new student who is required to be covered by health and accident insurance under University policy but wish to waive participation in the Student Health Insurance Plan, you must complete a Waiver Form which indicates you have comparable coverage from another source. The Waiver Form must be completed online by August 15, 2009. The online waiver allows students to quickly and easily waive participation in the Plan.
To complete the online waiver:
- Log on to www.universityhealthplans.com
- Select UMDNJ from the list of schools.
- Select UMDNJ Student Health Insurance Plan.
- Select the Waiver Form from the MENU.
- Complete the Form. Be sure to accurately enter all required information. If you do not include all required information, the Form will not be accepted.
- Submit the Form. You will receive a confirmation that your Waiver Form was successfully completed. Please print a copy for your records.
The online Waiver Form must be completed by August 15, 2009 to avoid being automatically billed for the Student Health Insurance Plan or being non-compliant with UMDNJ policy.
If you are a continuing student and are already enrolled in the Student Health Insurance Plan....
- If you are a continuing student who pays tuition directly to UMDNJ and are currently enrolled in the Student Health Insurance Plan, you will automatically be enrolled for Academic Year 2009-2010 and will be billed for the Student Health Insurance Plan on your UMDNJ tuition bill. You do not need to complete an Enrollment Form. However, if you wish to enroll eligible dependents, you must complete a Dependent Enrollment Form. This Form will be available at your School or you can download a Form at www.universityhealthplans.com. The completed Form and premium payment must be sent to our service provider, University Health Plans, One Batterymarch Park, Quincy, MA 02169. The deadline for enrolling dependents is August 15, 2009.
- If you are a continuing student currently enrolled in the Student Health Insurance Plan, but pay tuition to one of UMDNJ's partner institutions that co-sponsors a program with UMDNJ, you must complete another Enrollment Form for Academic Year 2009-2010. This Form will be available at your School or you can download a Form at www.universityhealthplans.com. The completed Form and premium payment must be sent to University Health Plans, One Batterymarch Park, Quincy, MA 02169. The deadline for enrolling is August 15, 2009.
- If you are a continuing student currently enrolled in the Student Health Insurance Plan and you are required to be covered by health and accident insurance under University policy, but wish to waive participation in the Student Health Insurance Plan for Academic Year 2009-2010, you must complete a Waiver Form which indicates you have comparable coverage from another source. If you are a continuing student and waived participation in the Student Health Insurance Plan last year....
- If you are a continuing student who is required to be covered by health and accident insurance under University policy, but you waived participation in the Student Health Insurance Plan last year and want to waive participation in the Plan again for Academic Year 2009-2010, you must complete an online Waiver Form by August 15, 2009. Otherwise, the Aetna insurance premium will automatically be placed on your UMDNJ tuition bill.
To complete the online waiver:
- Log on to www.universityhealthplans.com
- Select UMDNJ from the list of schools.
- Select UMDNJ Student Health Insurance Plan.
- Select the Waiver Form from the MENU.
- Complete the Form. Be sure to accurately enter all required information. If you do not include all required information, the Form will not be accepted.
- Submit the Form. You will receive a confirmation that your Waiver Form was successfully completed. Please print a copy for your records.

- If you are a continuing student who waived participation in the Student Health Insurance Plan last year, but you want to participate in the Plan for Academic Year 2009-2010 and you pay tuition directly to UMDNJ, you will automatically be enrolled in the Plan. However, if you pay tuition to one of UMDNJ’s partner institutions that co-sponsors a program with UMDNJ, you must complete the Enrollment Form. If you wish to enroll eligible dependents, you must complete a Dependent Enrollment Form. Both Enrollment Forms will be available at your School or you can download them at www.universityhealthplans.com.The completed Forms and premium payment must be sent to our service provider, University Health Plans, One Batterymarch Park, Quincy, MA 02169. The deadline for enrolling is August 15, 2009.
In addition, UMDNJ is pleased to offer our students and their dependents a Dental insurance plan effective August 1, 2009. You may enroll in this plan on a voluntary basis, it is not required insurance. The online enrollment form, plan benefit highlights, and a list of network dentists can be found by linking to www.universityhealthplans.com and selecting University of Medicine & Dentistry of New Jersey and then Dental. The deadline to enroll is July 15, 2009.
