Massachusetts Health Care Notice

The State of Massachusetts requires all residents to be insured. Proof of insurance coverage will be requested on your State Tax Return. Consolidated Health Plans will send a Health Care 1099 statement to each insured student at his/her local address. Do not throw this form out. You will need it to complete your 2007 Massachusetts Tax Return. Should you have any questions or have not received your MA 1099 HC by February 4, 2008, please contact University Health Plans via email info@univhealthplans.com.
