Delta Dental has several plan types. The plan offered to you is a DeltaCare USA DHMO Plan. 

When enrolling in the DeltaCare USA Dental Plan, you MUST select a Primary Care General Dentist (PCD) who participates in the DeltaCare USA network AND located in New York. Do not assume that a dentist who accepts “Delta” accepts all Delta Dental plans.

To find a participating DeltaCare USA dentist in New York:

• Enter a location in New York as your location
• Select “DeltaCare USA” as your plan. (See example image below.)
• If you do not select the DeltaCare USA Plan and a location in New York before searching for a provider, the dentists who appear on the results page may not be considered in-network for this plan.
