The final spring 2024 semester deadline to enroll dependents was February 29, 2024. This form is no longer available.

If your dependent has experienced a qualifying life event (ie. loss of other coverage, recent marriage, birth, etc.) they may be eligible to enroll outside of the open enrollment period. Please contact University Health Plans with any questions regarding dependent enrollment or qualifying event enrollment at info@univhealthplans.com or 833-251-1129.
