VOLUNTARY STUDENT ENROLLMENT FORM
Voluntary Students who are approved by The University of Delaware and confirmed eligible, may purchase the UD Plan. Students who are approved and confirmed eligible for the plan but who will not be returning to UD in Spring 2024, due to graduation or other reasons, are able to purchase Fall Only Coverage (8/15/23-1/31/24).
Annual Enrollment Students: This plan IS NOT sold on a per semester basis, you must purchase the annual coverage by September 15, 2023, unless otherwise specified. If Fall Only coverage is purchased your plan will terminate on 1/31/24 and you will not be eligible to purchase Spring 2024 coverage.
Do not purchase this plan if your eligibility for enrollment has not yet been confirmed. Reach out to info@univhealthplans.com to confirm eligibility. Your enrollment will be removed and will not be processed if you do not have confirmed eligibility prior to purchasing. Non-contracted sustaining students are not eligible for this plan.
