FIND A PROVIDER INSTRUCTION

To find Blue Cross Blue Shield participating PPO providers in any state:

Steps for Finding Providers:

1. Log on to www.bluecares.com/healthtravel/finder.html
2. Members will then be prompted to either click on the “member” tab or the “guest” tab.
3. Click on the “guest” tab.
4. Visitors will also be prompted to “choose a product type.”
5. Select PPO when prompted to “choose a product type.”
6. Click “continue” at the bottom of the page.
7. Then enter the city and state of your location.
8. For a listing of providers in your area click “search by specialty” or enter the specific
   provider name (if searching by name) and click on “search by name.”
