Please review the below important notifications and reminders prior to completing the Curry College 2017 Spring Waiver Form.
- Out of State Health Maintenance Organization (HMOs), Exclusive Provider Organization (EPOs), and Out of State Medicaid Programs generally have a limited network of providers and do not provide coverage for non-emergency care in the local area surrounding Curry College.
- Travel medical insurance plans and international insurance plans are not comparable to the Curry College student health insurance plan and do not provide coverage other than emergency care in Massachusetts. Waiver forms will be reviewed by the Plan Broker and Administrator, University Health Plans. Waiver forms submitted with these types of plans will not be accepted.
Please read and review your plan summary of benefits before completing the Curry College 2017 Spring Waiver Form. If you are covered under your parent’s insurance plan and your parents do not live in Massachusetts, please call your health insurance company and confirm that your plan is comparable to the Curry College student health insurance plan and that you have access to comprehensive health insurance coverage in the Milton area. Comparable coverage is a health plan that provides comprehensive coverage for preventative care, emergency services, surgical services, hospitalization benefits, outpatient services and mental health services. Emergency coverage only is not sufficient.
