The 2025-2026 Annual Waiver Form will be available by the beginning of May. 

The 2024-2025 annual waiver form is closed. If you were eligible for the Emmanuel College Student Health Insurance Plan (SHIP) and did not submit the waiver or enrollment form by 1/1/25 you have been automatically enrolled in the Emmanuel College SHIP. Please email info@univhealthplans.com to request the waiver appeal form if you have other comparable coverage.
