2020-2021 Policy Year Waiver Form
(August 25, 2020 - August 24, 2021)
This document must be read before you may to continue to the waiver form.
A waiver must be submitted every Fall semester. Waiving the Fall semester’s fee removes the Spring’s semester’s fee.
By waiving the SVA-sponsored Student Health Insurance Plan, you are certifying that you have a current health insurance plan with medical coverage that meets or exceeds SVA’s requirements as indicated below.
You will receive an online receipt and email confirming submission of the waiver form, which you should retain as evidence that you have submitted the waiver form.
If your waiver is not accepted, you will receive an email confirming you will remain enrolled and the Health Insurance Fee will remain on your tuition bill. If you feel your waiver was declined in error, please contact University Health Plans at 1-800-437-6448 or email@example.com.
