Student Health Insurance Plan
2021-2022 Policy Year Waiver Form
This document must be read before you may to continue to the waiver form.
A waiver must be submitted every Fall semester. Waiving the Fall semester’s fee removes the Spring’s semester’s fee.
By waiving the SVA-sponsored Student Health Insurance Plan, you are certifying that you have a current health insurance plan with medical coverage that meets or exceeds SVA’s requirements as indicated below.
You will receive an online receipt and email confirming submission of the waiver form, which you should retain as evidence that you have submitted the waiver form.
Prior to the start date of your program, your student status will be reviewed to determine if you are approved to access campus facilities. If you will be accessing campus facilities and it is determined that the plan you waived with does not meet the requirements for coverage in the area of the school you will receive an email confirming that the insurance charge will be added back to your tuition bill and you will be enrolled in the Student Health Insurance Plan. If you feel your waiver was declined in error, please contact University Health Plans.
