UNITED CONCORDIA FLEX PPO DENTAL PLAN
2026-2027 Policy Year
Dear Students,
University Health Plans is pleased to offer a comprehensive dental option to students attending Lafayette College and their dependents. Enrollment in this plan is VOLUNTARY; it is not required insurance.
Students and dependents who wish to enroll in this plans MUST submit the online enrollment form and pay the full premium amount online via credit card no later than midnight on the posted deadline for their selected coverage period.
Enrollment will be processed AFTER the deadline and once processed, coverage will be effective September 1, 2026.

PARTICIPATING PROVIDERS
In-Network: This plan utilizes the Concordia Advantage Plus network which includes providers nationwide and offer discounted fees. Members do not need to select a primary care dentist under either network. You will receive the greatest out-of-pocket savings when visiting an Advantage Plus network provider.

Out-of-Network Coverage: The Concordia Flex PPO Plan provides coverage for services received from dentists who DO NOT participate in the Advantage Plus network however it is very important to know and understand that your out-of-pocket expenses will be greater. Please review the Plan Highlights & Benefits carefully.
