Eligibility
All registered International students taking credit hours must be enrolled in the plan on a mandatory basis (with the exception of those participating in ISEP). All registered Domestic Graduate students taking 6 or more credit hours and Domestic Undergraduate students taking 12 or more credit hours must enroll in the plan on a hard waiver basis.
Students (Names Insured, as defined in the Certificate) must actively attend classes for at least the first 31 days after the date for which coverage is purchased. Home study, correspondence, and online courses do not fulfill the eligibility requirements that the student actively attend classes.

The student health insurance plan (SHIP) is an ACA (Affordable Care Act) compliant plan insured by United Healthcare Student Resources (UHCSR) and includes access to United’s national PPO network. Additional plan information is located on the left side of this page.

Waiver Process
All students who are required to have health insurance will be charged for the SHIP. Students with comparable health insurance may waive SHIP coverage by submitting a waiver form by the applicable deadline. To be considered comparable, a plan must:
- Be filed and approved in the U.S. and compliant with the Affordable Care Act (ACA).
- Provide comprehensive benefits, including non-emergency benefits in the geographical area surrounding the Butler campus.
If the plan does not meet the requirements, students will be notified that their waiver is not accepted and will be enrolled in the SHIP. Please click the Waiver Form on the left for additional information about the waiver requirements and comparable plans.
Students who miss the waiver deadline will be enrolled in the SHIP and responsible for paying the insurance premium. Students who miss the annual waiver deadline are permitted to waive spring coverage.
Enrollment Process
Students that need coverage should enroll in the SHIP by completing the enrollment form on the left-hand side navigation menu. Students will not be automatically enrolled by the University until after the deadline date. Students should complete the enrollment form as soon as possible to avoid delays in accessing their health insurance benefits.
NOTE: UnitedHealthcare reserves the right to adjust the terms of the policy (i) in the event of any changes in federal, state or other applicable legislation or regulation; (ii) in the event of any changes in Plan design required by the applicable state regulatory authority; and (iii) as otherwise permitted in the our policy.
