Dear St. John's Students:
St. John's University is pleased to offer our students and their dependents a dental insurance plan (Delta Care USA). You may enroll in this plan on a VOLUNTARY basis; it is not required insurance. The on-line enrollment form, plan benefit highlights, and a list of network dentists can be found on the left side of this page.
The deadline to enroll is is August 15, 2026. Coverage will be effective September 1, 2026 through August 31, 2027.

ASSIGNING A PRIMARY CARE DENTIST
- This Plan is a Dental HMO plan. As part of the enrollment process, you (and any dependents you choose to enroll) MUST elect a Delta Care USA participating general dentist located in New York as your primary care dentist. Your enrollment form will not be accepted without this election.
- Use the "Provider Search" in the navigation menu on the left to locate participating providers.
- On the enrollment form list the facility ID number found under their name.

As with any insurance plan, be sure to review the Benefit Information BEFORE enrolling. The copayments on the Plan Highlights are subject to change. The most up-to-date copayment schedule can be found on this page at any time during the policy year.
