Indiana University
OPT Student
Insurance Plan
Enrollment Form
2025-2026 Academic Year
Students on OPT must maintain continuous active health insurance while in the U.S. and may enroll in the IU plan semester-by-semester for up to three years after graduation.
Students on OPT may enroll their eligible dependents, including the student's legal spouse and unmarried children under age 26 on a semester-by-semester basis.
If the student or dependent obtains other coverage through their employer, they may drop IU coverage after the close of a semester. However, no portion of the premium for a semester is eligible to be refunded, and coverage is not terminated early.
If the student does not enroll themselves or their dependent(s) during any open enrollment period, they may only enroll later if they have a qualifying life event and document the loss of comparable coverage. (Please note that comparable coverage is coverage from your employer. Plans marked to international students only are not comparable coverage.)
OPT Students must contact their campus international department for permission and access to enroll in this plan and receive the 2025-2026 access code.
