Qualifying Event Enrollment Form
2025-2026 Academic Year
A qualifying event, such as losing comparable health insurance, makes you eligible for a special enrollment period. This allows you to enroll in the student health insurance plan outside the open enrollment period.
To enroll in the MGHIHP Student Health Insurance Plan, you must submit this form within 60 days of the qualifying event. Enrollment will not be available after this deadline.
All qualifying event enrollment requests require supporting documentation, and eligibility will be verified with by MGHIHP before enrollment can be processed.
