STUDENT HEALTH INSURANCE PLAN
2026-2027 Academic Year
The 2026-2027 University of Toledo Student Health Insurance Plan (SHIP) is insured by Anthem Student Advantage. University of Toledo partners with University Health Plans to administer the waiver and enrollment process, as well as the dental and vision insurance enrollment. The SHIP complies with ACA benefit requirements and includes access to Anthem's national network of PPO providers. Students do not have to select a primary care provider and referrals are not required. Detailed plan information is located in the left-hand side navigation menu.
SHIP ELIGIBILITY
Domestic students taking 6 or more credits and international students taking 1 or more credits are charged for and enrolled in SHIP unless proof of comparable coverage is provided by completing the waiver form through University of Toledo. The waiver form is located in the left-hand side navigation menu. You must be logged in to the University of Toledo portal to access the waiver form.
All degree granting domestic students taking less than 6 credits are not charged for the SHIP on their student account and must complete an enrollment form through University Health Plans. The voluntary enrollment form is located in the left-hand side navigation menu.
All students enrolled in the plan have the option to enroll their dependents.

VOLUNTARY ENROLLMENT PROCESS
Degree-seeking domestic students enrolled in fewer than 6 credits are eligible to enroll in the Student Health Insurance Plan by completing the Voluntary Enrollment Form, located in the left-hand navigation menu. These students may also enroll eligible dependent(s) using the same form during the enrollment process.
DEPENDENT ENROLLMENT PROCESS
Students who are enrolled in the Student Health Insurance Plan and charged for coverage through their University of Toledo student account may enroll eligible dependent(s). To do so, please complete the Dependent Enrollment Form located in the left-hand navigation menu.
DENTAL PLAN
The dental plan offered for the 2026-2027 Policy Year is Delta Dental PPO Plan. Enrollment forms, premium rates, coverage periods, deadlines, plan benefits, and the provider search tool can be found in the Delta Dental of Ohio Plan section of the left-hand navigation menu. Enrollment is open to all degree-seeking students and their eligible dependents. Participation in the dental plan is voluntary and does not require enrollment in the Student Health Insurance Plan (SHIP).
VISION PLAN
The vision plan offered for the 2026-2027 Policy Year is Davis Vision. Enrollment forms, premium rates, coverage periods, deadlines, plan benefits, and the provider search tool can be found in the Davis Vision Plan section of the left-hand navigation menu. Enrollment is open to all students and their eligible dependents. Participation in the vision plan is voluntary and does not require enrollment in the Student Health Insurance Plan (SHIP).
