The voluntary enrollment form for the Fall 2026 period is no longer available. The deadline to voluntarily enroll in the U of Toledo Student Health Insurance Plan for the Fall 2026 period was on September 14, 2026. 

The Spring 2027 voluntary enrollment form will be available this fall.
