This plan is administered by Anthem Student Insurance Plans and University Health Plans (UHP).

Insurance Eligibility

This plan is available to all domestic Indiana University undergraduate and graduate students who are currently enrolled in classes (at least 1 credit hour of campus or online classes) and not eligible for other university-sponsored health insurance plans (such as the SAA plan, Fellowship Recipient plan, Professional Student Plan, International Student Plan, Visiting Scholar Plan, or an IU Employee plan).

If you are a Student Academic Appointee (SAA), an Appointed Post Doctoral Fellow or a Fellowship Recipient, you are automatically enrolled in the SAA or Fellowship Student Health Insurance Plan and should NOT complete this enrollment form. Please click HERE to be redirected to your plan.

If you are a domestic graduate student in any programs listed below, you are automatically enrolled in the Professional Student Health Sciences Insurance Plan and should NOT complete this enrollment form. Please click HERE to be redirected to your plan.

  • School of Medicine (IUSM),
  • School of Dentistry (IUSD),
  • School of Health and Human Sciences (SHHS),
  • School of Optometry (IUSO), or
  • Department of Speech/Language Pathology Master's & Doctor of Audiology (SLHS) program

  • The plan year begins August 1 and ends July 31 the following year
  • The Plan includes comprehensive medical, prescription, vision, and dental coverage for one premium
  • The medical plan uses the Anthem Blue Access PPO network with access to providers throughout Indiana, nationwide and overseas.
  • Deductibles and out-of-pocket amounts restart annually on August 1
  • Annual deductible is $350 per member / $1,050 per family for in-network coverage
  • Out-of-pocket maximum is $5,000 per member / $10,000 per family for in-network coverage
  • In-network preventive care is covered by the Plan at 100%
  • IU Student Health Center office visit copay is $15
  • Primary Care office visit copay is $25; Specialist office visit copay is $35
  • Urgent Care copay is $50; Emergency Room copay is $100
  • Most other services are covered with 20% coinsurance after deductible
  • Medical coverage includes therapy services such as chiropractic care, physical therapy, behavioral health, mental health, and substance use disorder services; See student brochure or plan booklet for more information
  • Vision care coverage through Anthem Blue View Vision, includes an annual eye exam with a $20 copay and discounts for vision wear
  • Dental care coverage through Anthem Blue Cross and Blue Shield Dental Complete, includes diagnostic and preventive services covered at 100% when using in-network providers, 50% coinsurance for most other services, and a $500 annual benefit maximum

  • Your medical coverage follows you wherever you go, including nationwide and overseas. As an Anthem member, you have access to care across the country through the BlueCard PPO program, and outside the U.S. through the BCBS Global Core program.   • Your health plan includes access to emergency travel assistance through Academic Emergency Services (AES). With AES you have access to 24/7 support if you experience an emergency while traveling over 100 miles from home or outside your country. Services include emergency medical evacuation and repatriation, pre-travel and safety assistance, and an accidental death & dismemberment (AD&D) benefit. Enrollment in this plan is open to all domestic Indiana University undergraduate and graduate students who are currently enrolled in classes and not eligible for other university-sponsored health insurance plans (such as the SAA plan, Fellowship Recipient plan, Professional Student Plan, International Student Plan, Visiting Scholar Plan or an IU Employee plan).

Eligible students may insure their dependents, including their spouse and unmarried children under the age of 26, for an additional cost. Documentation to support dependent relationship is required at time of enrollment (i.e. marriage certificate or birth certificate). Dependent eligibility is effective and expires concurrently with that of the insured student, meaning coverage is only available to dependents if and when the student is insured under the plan.

Coverage can be elected on a semester-by-semester basis or for the full academic year. Premiums can be paid by credit card for the entire coverage period at the time of enrollment. Premiums must be paid directly to UHP and cannot be charged to your bursar account. To enroll, select and complete the Enrollment Form from the left hand menu prior to the enrollment deadline listed below.

If the enrollment deadline has passed, you cannot enroll in this plan until the next coverage period unless you or your dependent(s) experience an IRS-qualifying life event, such as marriage, birth of a child, or loss of other coverage.

Once enrolled, no portion of the premiums are eligible to be refunded.

Students and dependents who do not enroll during the enrollment period may qualify for special enrollment rights if there has been a significant life change known as an IRS-qualifying life event (i.e. marriage, birth of a child, loss of other coverage, etc.). For students or dependents with a qualifying event, complete the qualifying event enrollment form and provide supporting documentation within 30 days of the life event.

Premiums cannot be charged to your bursar account. Coverage can be elected for a semester or academic year. Premiums can be paid in full at the time of enrollment.

After enrollment, you’ll receive an email from Anthem Student Advantage when your digital ID card is ready to download. You can access your ID card by downloading the Sydney Health app or visiting Anthem.com. You can then email or fax it from your smartphone or computer, or request a hard copy ID card be mailed to your address on file.
