Voluntary Insurance Plan Options
Dear Students,
Midwestern University is pleased to inform you that we have partnered with University Health Plans to offer students and their dependents the opportunity to purchase any or all of the following voluntary insurance plan options. Enrollment in these plans is VOLUNTARY and is not required by the University.
As with any insurance plan, please make sure to review the benefits of each plan prior to enrolling. Benefit information is subject to change. The most up-to-date benefit information can be found on this page at any time during the policy year.
Delta Dental PPO Plus Premier Plan
PARTICIPATING PROVIDERS: This plan utilizes the Delta Dental PPO and Delta Dental Premium networks which include providers nationwide and offer discounted fees. You do not need to select a primary care dentist under either network. You will receive the greatest out-of-pocket savings when visiting a Delta Dental PPO or Delta Dental Premier providers, respectively. Please be sure to review the Plan Summary for benefit information.

DeltaVision Plan -
PARTICIPATING PROVIDERS: This plan utilizes the EyeMed Select Network. You may choose to go to any licensed optometrist, ophthalmologist and/or dispensing optician whenever you need vision care, however there is significant cost advantages when you receive treatment from an in-network provider. You will only receive in-network benefits from EyeMed Select Network providers. Have your DeltaVision information card available when scheduling and visiting an in-network provider. Please be sure to review the Plan Summary for additional explanation and coverage information and the Provider Search function to find in-network providers.

ID cards are not issued for the Vision Plan. Once active, tell the vision provider that you are enrolled in EYEMED DELTAVISION COMPLETE SELECT. The Provider will verify eligibility using the enrollee name and date of birth. EyeMed Vision Care can be reached at 1-866-723-0513

University Health Plans is pleased to offer Renters Insurance that is made easy for you. GradGuard offers a variety of coverage options to meet your lifestyle and is able to deliver proof of coverage straight to your landlord.
