The 2025-2026 Annual Dependent Enrollment form is closed. The deadline to enroll was October 10, 2025.  

The dependent enrollment form for new spring students will be open in November. 

If you are a current student and your dependent(s) have experienced a qualifying event, such as a loss of other coverage, they may qualify for special enrollment rights. Please contact University Health Plans at 833-251-1123 or info@univhealthplans.com.
