The deadline to enroll in the subsidized voluntary dental plans for the 2025-2026 policy year effective September 1, 2025 was October 10, 2025. This was the final enrollment period. This form is no longer available.

If you are a student for spring 2026 semester and would like to enroll in a voluntary dental plan you may do so by clicking HERE. The deadline to enroll for coverage effective January 1, 2026 is December 10, 2025 and you are responsible for the full premium due.

The spring 2026 policy period enrollment form will be available in early November 2025.
