The deadline to confirm enrollment for the spring 2024 policy period was January 15, 2024. The form is no longer available. If you are an eligible new student for spring 2024 and did not fill out the waiver form by the deadline you will be automatically enrolled in the Student Health Plan retroactive to January 15, 2024 and responsible for the insurance charge on the student account.

If you previously submitted a waiver form and have experienced a qualifying event, such as a loss of other coverage, you may qualify for special enrollment rights. Please contact University Health Plans at info@univhealthplans.com or 1- 833-251-1152 for more information.
