The deadline to confirm enrollment for the spring 2026 policy period has passed. The form is no longer available. If you are a newly eligible student for spring 2026 and did not fill out the waiver form by the deadline you will be automatically enrolled in the Student Health Plan effective January 15, 2026 and responsible for the insurance charge on the student account.

If you previously submitted a waiver form and have experienced a qualifying event, such as a loss of other coverage, you may qualify for special enrollment rights. Please contact University Health Plans at info@univhealthplans.com or 1- 833-251-1152 for more information.
