The 2026-2027 enrollment form is closed. The Spring 2027 enrollment form is available only to newly eligible spring students and will open in late November. 

If you are a current student who waived at the start of the academic year and you have a qualifying event, such as loss of other coverage, please choose the Qualifying Event Enrollment Form from the menu on the left.
