A qualifying life event is a change in circumstances—such as losing other health coverage—that allows you to enroll through special enrollment rights. Students eligible for the Post University Student Health Insurance Plan (SHIP) may use this form to request enrollment. All eligibility will be verified before enrollment is finalized.
Loss of Other Coverage

Required Documentation: A loss of coverage letter showing your name and your last day of coverage.

Deadline: Within 31 days of your last day of coverage under the prior plan.

Effective Date of Coverage: Begins the first day you no longer have coverage under your prior plan.
