The spring 2026 policy period waiver deadline has passed. This form is no longer available. If you are an eligible new student for spring 2026 semester and took no action to confirm enrollment or waive, you willl be automatically enrolled in the Plan.

The 2026-2027 policy year waiver form will be available the week of June 15, 2026.
