Possible Reasons You Can't Access The Form:

• You are not a NEW Spring student.

• Only students who are required to have health insurance can access this form. You must fall into one of the below categories, and if you recently registered it may take up to 3 business days for your information to update this system. Check back next week.

  • Undergraduates taking 12 or more credits
  • Graduate students taking 9 or more credits
  • Biology/Biotechnology Graduate taking 8 or more credits
  • Doctor of Clinical Psychology taking 6 or more credits
