The waiver form for the 2025-2026 school year is closed. Students who did not complete the waiver form have been automatically enrolled and charged the student health insurance premium.

To access your online insurance card, please go to www.universityhealthplans.com/montclair and select Insurance ID Card on the left side of the page.

The waiver form for Spring 2025 will be available early December.
