The 2025-2026 spring waiver form is no longer available. The deadline to complete the waiver was February 3, 2026. Students who did not take action by the posted deadline were given until February 26, 2026 to complete a waiver. Waivers are not permitted after the final deadline extension has passed.
As a reminder, a waiver form must be submitted every academic year by the posted deadline. Your waiver form does not carry over year to year.
Effective August 15, 2025, the insurance carrier for the 2025-2026 Seton Hall University Student Health Plan will be Aetna Student Health. Please make sure that your current providers are participating with Aetna. To search the Aetna provider directory please use the provider search HERE. When asked to select a plan, under "Student Health Medical Plans" select Open Choice PPO.

Before accessing the enrollment form, read the following and acknowledge the statements below.
A qualifying event is an event such as losing comparable health insurance coverage or entry into the USA. These changes may make you eligible for a special enrollment period, allowing you to enroll in the student health insurance outside the open enrollment period. You can use this form to enroll for coverage on the Seton Hall University Student Health Plan within 60 days of the qualifying event date. Enrollment is not available after 60 days following the eligible life event.
Important: When enrolling your effective date of coverage MUST match the first day of no coverage under your other insurance, if enrolling due to a loss of coverage. Your enrollment WILL NOT be processed if the effective date does not match your supporting documentation and your enrollment will NOT be processed, and you will be refunded. If you have questions about the coverage start date, please contact University Health Plans at 833-336-2121 before you submit your enrollment form.
You must enroll for your applicable student coverage type: Undergraduate, Graduate or Law Student. If you are a graduate or law student and select to enroll as an undergraduate student, your enrollment will NOT be processed, and you will be refunded.
