Plan Highlights
Wake Forest University School of Medicine (WFUSM) requires that all students have health insurance. WFSM has made arrangements for students and their families to purchase insurance at group rates. Students required to have health insurance will be enrolled in and charged for the Student Health Insurance Plan (SHIP). The annual insurance cost will be split into two charges. Students may opt-out of the SHIP by submitting a waiver form documenting proof of comparable coverage at the start of the academic year. Once the waiver deadline has passed, students cannot opt-out of the insurance plan.
Before opting out of the student health insurance plan, please check your individual plan to confirm their participation in the local area. View this list of common plans accepted by Wake Forest Baptist. Due to the different physician groups and hospitals within the Wake Forest Baptist system, physician services and hospital services are billed separately. Please remember that health insurance coverage varies, so some services may not be covered.

Waiver Process
Students covered by a comprehensive health insurance plan in North Carolina can opt-out of the SHIP by completing the waiver form. Before waiving, compare the cost, coverage, and benefits of the SHIP to your existing plan. If you do not submit a waiver form by the deadline stated above, you will be enrolled in the SHIP and responsible for the cost of the insurance plan.
Dependent Enrollment Process
If you are enrolled in the Student Health Insurance Plan, you may also purchase coverage for your eligible dependents by completing the enrollment form in the navigation menu on the left side of this page. You will be prompted to make an online payment with a credit card.
Qualifying Event Enrollment
Once you waive the SHIP, you cannot voluntarily enroll. However, students who waived the SHIP may qualify for special enrollment rights if they incur an involuntary loss of other coverage or acquire a new dependent. To request an enrollment form and premium quote, contact University Health Plans at 800-437-6448 or info@univhealthplans.com. You must submit all qualifying event enrollment paperwork and documentation within 30 days of the qualifying event.
