STUDENT HEALTH INSURANCE PLAN

Dear Rutgers Student and Parents:
Rutgers University is pleased to present you with information about the Rutgers University Student Health Insurance Plan. The Student Health Insurance Plan (SHIP) is serviced by University Health Plans (UHP) and underwritten by UnitedHealthcare Insurance Company (UHC).
Full Time Domestic Students
Fall/Spring Enrollment Eligilbility
All full time Undergraduate Students taking 12 or more credit hours and all full time Graduate Students taking 9 or more credit hours* are required to have comprehensive Health Insurance each semester. Student insurance eligibility may be affected if an enrollment status change occurs (part time to full time or full time to part time). Students must actively attend classes for at least 31 days after the date for which coverage period is purchased.
*full time students as defined by the student’s department could require you to be registered for 12 credits, example: Rutgers Business School, Rutgers Law School.
Summer Enrollment Eligilbility
All new full time Undergraduate Students registered for 6 or more credit hours and all full time Graduate students registered for 4.5 or more credit hours in the summer are eligible to enroll in the Full time Student Health Insurance Plan for Summer.
The Student Health Insurance premium is added to your Rutgers term bill each semester once you are registered for Full Time credits. You are automatically** enrolled after the deadline in the Student Health Insurance Plan unless comparable coverage is provided. You must waive online, before the deadline, at www.universityhealthplans.com to void your automatic enrollment.
**Exception: Graduate Fellows, who are eligible as determined by the Graduate School, and Partial appointment Teaching and Graduate Assistants-No Waiver action required. UNLESS you are an eligible Graduate Fellow and you decline the Graduate Fellow health insurance, then you must waive the student health insurance. Full appointment Teaching and Graduate Assistants MUST waive each semester.
When you access your term bill for the first time each semester, in the pop up window, you will select whether you intend to waive or enroll. This only indicates your intention. The enrollment or waiver process is still incomplete. You must complete the waiver or enrollment form online at www.universityhealthplans.com.
WAIVING: The charge will be removed from your term bill within 4 business days after receiving a waiver conformation number. Failure to waive online before the deadline will result in a non-refundable charge and students will be responsible for the premium even if you have other coverage.
ENROLLING: We encourage you to submit the enrollment form before the deadline. Insurance cards are available 5 -7 business days after you receive your enrollment confirmation number.
To add dependents or a spouse, you must visit www.universityhealthplans.com. Payment is made directly to University Health Plans. The premium for dependents/spouse cannot be added to the Rutgers Term Bill.
When your enrollment or waiver form have been successfully submitted you will receive a confirmation number on the screen and to your Rutgers email address. If you did not receive a confirmation number or for enrollment or waiver issues, call University Health Plans directly at 800-437-6448.
NOTE: You will get an email from UnitedHealthcare StudentResources (www.uhcsr.com) within 5 - 7 business days after enrollment with instructions to print a card. Please call UnitedHealthcare StudentResources directly at 800-505-4160 if you need assistance in this process.

Part Time Domestic Students
All part-time degree seeking students taking at least 1 credit hour are eligible to enroll on a voluntary basis. Insured part time students may purchase coverage for their dependents on a voluntary basis at www.universityhealthplans.com. Payment is made directly to University Health Plans for either students or dependents and cannot be added to the Rutgers Term Bill.
When your enrollment form has been successfully submitted, you will see a confirmation number on the screen, and will also receive an email to your Rutgers University email address shortly thereafter.
Rutgers University F1, F2, J1, J2 Visa Sponsored Students
Students are automatically enrolled each semester by the Center for Global Services and the charge is added to your term bill. To add dependents or spouse, you must contact the Insurance Coordinator at http://globalservices.rutgers.edu.
If you have other coverage which meets specific requirements and wish to waive, you must contact the Insurance Coordinator at http://globalservices.rutgers.edu. For waiver requirements visit http://globalservices.rutgers.edu/content/Center_Staff_Services_and_Programs/Health_Insurance_Program/Rutgers_Health_Insurance_Requirement.html. Failure to waive before the deadline will result in a non-refundable charge, and students will be responsible for the premium on their term bill. NOTE: The Student Health Insurance premium is a separate charge from the Health Services Fee.
Non-Rutgers University F1, F2, J1, J2 Visa Sponsored (International) students must follow the applicable process as shown above for full time or part-time domestic students.
