The final deadline to confirm enrollment for the spring 2026 policy period has passed. This form is no longer available. If you are an eligible new student for spring 2026 semester charged for the insurance and did not waive, you will be automatically enrolled in the plan.

You may qualify for special enrollment rights if you previously waived the plan and recently lost your other coverage. Please contact the University Health Plans at info@univhealthplans.com for enrollment information.
