The Dependent Enrollment Form is not currently available. The deadline to enroll dependents for annual coverage was on September 15, 2023.
Contact University Health Plans (info@univhealthplans.com or 833-251-1127. immediately if your dependent recently experienced a qualifying event (such as the dependent's: entry into the U.S., marriage to student, official same-sex civil union partnership status with student, birth, adoption, or loss of other coverage). The deadline for submitting all required items to UHP to an eligible dependent to your UD Plan following a recognized qualifying event is 31 days from the date of the event.
