The spring 2026 policy period voluntary enrollment deadline was February 23, 2026. The enrollment form is no longer available.

If you have a life changing event (i.e. involuntary loss of coverage) and need to enroll in the insurance plan, please contact University Health Plans at 1-833-251-1148 or info@univhealthplans.com. Enrollment must occur within 30 days of the qualifying life event.
