The spring 2024 waiver deadline was February 22, 2024. The spring waiver appeal deadline was March 22, 2024. The form is no longer available. A late waiver will not be accepted at this time.

The 2024-2025 policy year waiver form will be available in mid-July 2024. Please check back in early summer for additional information and any updates.
