Spring Waiver Appeal Form

The spring 2026 waiver deadline has passed.

If you registered as an undergraduate student for 9 or more credits for spring 2026 semester, were charged the spring insurance fee on your tuition bill and did not fill out the waiver form by the spring 2026 deadline, February 23, 2026, you have been automatically enrolled in the student health insurance plan. The spring semester insurance charge, $660, is included on your tuition bill.

If you feel that you have an extenuating circumstance for missing the spring 2026 waiver deadline and are enrolled in another insurance plan with comparable coverage and effective date prior to January 15, 2026 you may be eligible for a late waiver. 

The deadline to submit a late waiver appeal is March 20, 2026.This deadline is final.
