The fall 2025 enrollment deadline has passed. If you were charged the fall 2025 insurance fee on your student account and did not submit a waiver form by the deadline, you will be automatically enrolled in the Plan.

If you have a life changing event (i.e. involuntary loss of coverage) and need to enroll in the insurance plan after waiving, please contact University Health Plans at 1-833-251-1148 or info@univhealthplans.com. Enrollment must occur within 30 days of the qualifying life event.

The spring 2026 policy period enrollment confirmation form will be available in early December 2025.
