The 2025-2026 Enrollment Form is now closed. If you are eligible for the Student Insurance Plan and did not yet submit the waiver form then you have been automatically enrolled in the student health insurance plan. 

If you have experienced a qualifying event, such as a loss of other coverage, you may qualify for special enrollment rights. Please contact University Health Plans at 833-251-1151.
